SOUTHWEST Airlines suffered a catastrophic failure mid-air after Flight 1380’s engine exploded, causing the death of one passenger onboard. Images of the cabin have led a former-flight attendant to suggest that many didn’t wear the oxygen masks correctly in the emergency.
